- Fill out a Claim form [PDF Format, 80 KB - Help] available from retailers.
- For prizes over $600, attach a photocopy of one of the following valid forms of identification: health insurance card, driver's permit or passport. The expiration date must be visible. You also need to include the duly filled out questionnaire on the Claim Form.

- At a retailer
For all lottery games, prizes of $600 or less may be claimed from a retailer.
It will be important to verify that the retailer has the necessary cash on hand to pay your prize before validating your ticket. Once the ticket has been validated, you may claim your prize ONLY FROM THAT RETAILER or from Loto-Québec. Be sure that the retailer returns the ticket to you as well as your ticket validation coupon.
- At Loto-Québec counters (no appointment required)
